Assessment Privacy Notice
This Assessment Privacy Notice explains how The Creative Mill LLC, a Louisiana limited liability company doing business as Aligned for Impact (“Aligned for Impact,” “AFI,” “we,” “us,” or “our”), collects, uses, discloses, and safeguards information submitted through the AFI Impact Accelerator Baseline Assessment.
By submitting the assessment, you acknowledge the practices described in this notice. If you do not agree, do not submit the assessment.
01 Scope
This notice applies only to information collected through the Impact Accelerator Baseline Assessment, its saved results pages, and the private AFI assessment dashboard. It does not govern other Aligned for Impact websites, programs, forms, payment services, or third-party platforms that may have their own privacy notices.
02 Information We Collect
We collect information that you provide directly through the assessment, including:
- Your first and last name and email address.
- Your numerical self-ratings across the Wheel of Alignment and Accelerator Baseline.
- Your written responses about goals, priorities, commitments, capacity, barriers, and support needs.
- Dates, status information, and resources connected to commitments you identify.
- Your confirmation that you reviewed the responses and consented to their storage and use.
Our hosting and security providers may also process limited technical information needed to deliver and protect the site, such as IP address, browser or device information, request logs, and timestamps.
03 How We Use Information
We use assessment information to:
- Prepare for and guide your Impact Accelerator sessions.
- Understand your current priorities, alignment, capacity, and support needs.
- Create and display your assessment summary, Wheel of Alignment, and downloadable report.
- Organize and manage client records and program delivery.
- Maintain, troubleshoot, secure, and improve the assessment experience.
- Comply with legal obligations and protect the rights, safety, and integrity of AFI, our clients, and our services.
We do not use assessment responses for targeted advertising or sell or rent them to third parties.
04 How We Disclose Information
Assessment information may be disclosed only as reasonably necessary to:
- Authorized Aligned for Impact personnel who support your engagement.
- Service providers that host, store, secure, or maintain the assessment system and that process information on our behalf.
- Government authorities, courts, or other parties when required by law or reasonably necessary to protect legal rights, prevent harm, investigate misuse, or respond to a security incident.
- A successor organization in connection with a merger, acquisition, reorganization, or transfer of business assets, subject to applicable law.
Submission notification emails, if enabled, are intended to contain basic identifying and completion information rather than your full written responses or attached report.
05 Retention
We retain assessment information for the duration of your client relationship and for a reasonable period afterward when needed for program delivery, client service, business records, dispute resolution, security, or legal compliance. We may retain limited information longer when required or permitted by law. When information is no longer reasonably needed, we may delete it or remove identifying details.
06 Security
We use reasonable administrative and technical safeguards designed to protect assessment information, including restricted administrative access and protected data storage. No online system can guarantee absolute security. Please keep your saved results link private and contact us promptly if you believe it has been accessed without permission.
07 Your Choices
You may contact us to request access to, correction of, or deletion of your assessment information, or to withdraw consent for future use. We may need to verify your identity before completing a request and may retain information when required or permitted by law. Withdrawing consent does not affect processing that occurred before the withdrawal.
Depending on where you live, applicable law may provide additional privacy rights. We will review and respond to verified requests as required by applicable law.
08 Children’s Privacy
This Impact Accelerator assessment is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through it. If you believe a child has submitted information, please contact us so we can review and address the request.
09 Changes to This Notice
We may update this notice as our assessment practices, service providers, or legal obligations change. The “Last Updated” date above will identify the current version. Material changes will apply prospectively unless otherwise permitted by law.
